General Forms and Applications
Building Use
Public use of school facilities may take place during and after school hours if the use will not be disruptive to normal school operations. The Board authorizes the use of school facilities, both indoor and outdoor.

Census
The Schoharie Central School District is taking a census of all school district residents with children and we need your help. The purpose of this census is to assist us in planning for future student enrollment. Please take a moment and list all children who reside at your address, including children under age five (5) and up to age eighteen (18). Please be sure to include all students attending any public, private, parochial, or home schools. All information will be held in strict confidence.
Dental Health Certificate
New York State public schools ask for a dental health certificate from students at the time of school entry and in grades K, 2, 4, 7, and 10. The Dental Health Certificate can be downloaded from the New York State Education Department website.
Free and Reduced Price School Meals
To apply for free and reduced price meals for your children, read the instructions on the back, complete only one form for your household, sign your name and return it to:

Harassment Reporting
The Schoharie CSD maintains a policy prohibiting all forms of discrimination, offensive behavior and harassment or violence against students or employees. All persons are to be treated with respect and dignity. Sexual violence, sexual advances or other forms of harassment by any person which create an intimidating, hostile or offensive environment will not be tolerated.

NYS Health Examination Form
The State Education Department requires a physical exam for new entrants and students in Grades Pre-K or K, 1, 3, 5, 7, 9 & 11; annually for interscholastic sports; and working papers as needed; or as required by the Committee on Special Education (CSE) or Committee on Pre-School Special Education (CPSE). The Required Health Examination Form can be downloaded from the State Education Department website.
Transportation to a Non-Public School
School age children will be provided transportation to private/parochial schools within 15 miles of the child’s home. Such transportation must be requested in writing, by April 1, preceding the school year which the transportation is needed, or within 30 days of moving into the school district. Parents MUST submit for transportation each year their pupil is enrolled in private or parochial school.
Wall of Fame Alumni Nomination
The Schoharie Central School District Alumni Wall of Fame Award has been established to recognize graduates of Schoharie High School who have made a significant contribution to society. Qualifying candidates should have high moral character, have exhibited good citizenship, made a lasting contribution to society, demonstrated outstanding leadership, and be at least a 15-year high school graduate.
Working Papers
School districts in New York State are responsible for issuing employment certificates and permits to qualified applicants. Minors may apply for certificates where they live, where they go to school, or where they are to be employed. Working Paper forms can be downloaded from the New York State Department of Labor website.

Athletics
Students who want to play sports must submit health forms as well as a Late Bus Request and Sports Alternative Pickup Note. Access all Athletics forms on our Athletics Forms page.
